Insurance A referral or prescription from your medical provider is required before we can begin. We will help with authorization and coordination once you have it.

We have sample prescription forms for OT, SLP, and PT that you can bring to your provider to make the referral process easier. The completed form can be sent to us via fax: 866.436.7605.
