Services may include occupational therapy (OT), speech-language pathology (SLP), and physical therapy (PT). Care supports regulation, communication, movement, participation, and everyday functioning in ways that honor each person’s strengths, sensory needs, and developmental profile.

To begin therapy services at Empowering the Best, the next steps depend on whether you plan to use private pay or insurance.

If Using Private Pay

A prescription or referral is not required.You may begin services directly. If you have questions about choosing private pay versus insurance, please feel free to reach out.

If Using Insurance

A signed referral or prescription from a medical provider is required to use insurance for therapy services, including occupational therapy, speech-language pathology, and physical therapy.

What to Do

1. Download the appropriate referral form below.

2. Bring the form to your primary care provider, pediatrician, or specialist.

3. Ask the provider to complete the diagnosis section, check any relevant boxes as appropriate, and sign the form.

4. Please bring the completed form to your first appointment.
