If Using Private Pay
A prescription or referral is not required.
You may begin services directly.
If you have questions about choosing private pay vs. insurance, feel free to reach out.
If Using Insurance
A signed referral or prescription from your child’s medical provider is required to use insurance for occupational therapy services.
What to Do

Bring it to your child’s pediatrician, primary care provider, or specialist.
Share the form with the provider so they may complete the diagnosis section, check any relevant boxes if appropriate, and sign the form at their discretion.
